    Identify the premises the notice is about.

  3. Who inspected

    Inspections should be conducted when the premises are vacant unless the landlord and tenant or their agents otherwise agree.

  4. Condition of the premises

    The report describes the condition of the premises (Residential Tenancies Act, s. 19(1)-(2)).

  5. Signatures
